Surgery Center

What To Bring to the Surgery Center

  • A valid I.D. and your most current insurance card.

  • Current medication list with the strength of each medication and the frequency of the doses.

  • Your most current imaging and test results. These may include MRI or CT Scan results. The written reports and the films/CDs are recommended.

  • Bring a driver to drive you home if you will be receiving sedation.

  • Wear comfortable clothes to wear home post- procedure.

  • Please consult your facility for complete instructions.

Clinical Center

What To Bring to the Clinical Center

During your initial office visit, our goal is to work together with you to define your diagnosis and to construct a treatment plan to begin to restore your function. To help facilitate this treatment we will need you to bring the following:
  • Your most current imaging and testing results. These may include a MRI, CT Scan, or X-ray. The written reports and the films/CDs are recommended.

  • A valid photo I.D. and the most current insurance card.

  • Current medication list with the strength of each medication and the frequency of the doses.

  • Copy of the written referral if you were referred by a physician. (We do accept new patients who are either self-referred or referred by current patient)

  • Copies of previous procedures done if you have treated by another pain provider in the past.
