APAC Rehabilitation & Pain Management Services, along with its management team, APAC Partners, is the largest organization of its kind in the Chicagoland area and Northwest Indiana. Working with our clinical management team, APAC Partners, serves over 30,000 patients per year in various settings, including hospital-based, skilled nursing facilities, and Long-Term Acute Care facilities. APAC has been working clinically and operationally to meet the challenges of escalating healthcare costs and the need for evidence-based outcomes.  

Our team works in close cooperation with medical leadership in every kind of facility to ensure excellent clinical outcomes and appropriate documentation in the most cost-effective manner. These involve: evidence-based clinical pathways to optimize therapy programs, weekly team conferences to ensure continued integrated care among the entire rehabilitation team & administrative staff, and referring physician communication.

We are a Physiatry and Pain Management-based physician group. Our physicians ensure a multi-disciplinary approach to manage physicial therapy programs, treat patients with disabilities, in addition to providing interventional medicine, that result from injury or disease. We focus on improving patient’s functional capacity and minimizing a patient’s level of pain. We treat patients in every possible setting, from acute-care hospitals to assisted living, as well as our numerous outpatient facilities.

APAC provides a wide variety of services designed to treat your pain condition with a multidisciplinary approach, as well as diagnosis, medical managment, and rehabilitation of disabilities, resulting from injury or disease. Our pain team consists of experts in the fields of interventional anesthesiology, physical medicine and rehab physicians, nurse practitioners, registered nurses, and medical assistants.

In addition to our team at APAC, we have hand-picked expert specialists such as physical therapists, pain psychologists, and spinal surgeons who compliment our multifaceted approach. Utilizing our experienced team approach, we treat a wide variety of acute and chronic pain conditions, and focus on maximizing a patient’s functional performance.

We utilize interventional approaches (injections) to treat painful conditions such as herniated disks, spinal stenosis, nerve injuries from trauma or surgery, occipital headaches, shoulder pain, arthritic joint pain, and sympathetic nerve pain. We also provide surgical interventions such as spinal cord stimulators, discographies, and the cutting edge Trufuse procedure for facet joints in the spine.

Complimentary to the interventional approach, APAC’s comprehensive pain management program includes a full spectrum of treatments that are non-interventional. Our multi-specialty approach includes the use of medications, physical therapy and various modalities with the goal of improving your function and quality of life as well as reducing your pain.

Our team is experienced in managing musculoskeletal injuries, including acute & chronic neck & back pain, fibromyalgia, peripheral neuropathy, spinal stenosis, sacroiliac joint dysfunction, RSD, sprains & strains, arthritis, sports injuries, and headaches. At pain management center in Indiana, we treat patients in various settings from Skilled Nursing Facilities, long-term accute care, and in-patient/out-patient facilities.

The intensive rehabilitation program consists of coordinated medical and rehabilitation services, provided 7 days a week. The individual needs of the persons served, drive the appropriate use of the rehabilitation continuum of care offered at the sub acute and acute rehabilitation facilities.
